Step 1. Required Documentation
- Complete application - found HERE -
- Copy of Deed to Property (to document ownership) or ;
- If you rent, a signed landlord rental agreement. - found HERE -
- Copy of last 12 months utility bills (PSO, Choctaw Electric and Oklahoma Natural Gas)
- Proof of income:
Applicant and all household members with earned income must present pay stubs showing 30 days of wages (current pay stubs)
or
Non-working applicants and all household members with income from such agencies as: Social Security Administration, Department of Human Services, Veterans Administration, Retirement, and/or Pensions must present documentation of income i.e. such as letter from SSA.
A COMPLETE APPLICATION WITH ALL REQUIRED DOCUMENTATION MUST BE RETURNED TO THE APPROPRIATE COUNTY WHERE YOU RESIDE TO BE PLACED ON THE WAITING LIST.
